Project Application for Research with Human Subjects
All human subject research conducted by Texas A&M employees or students is required to be submitted to the Institutional Review Board for review. Once submitted, applications are routed to appropriate reviewer, based on data collection methods and level of risk, by the Human Subjects Protection Program.
Please note that most forms are now ONLY available in the iRIS program. For instructions and questions about the system, visit the iRIS Portal. If you need help with iRIS, call the support line at 979.845.4969.
Forms that are still available on this page were revised for use beginning September 1, 2011. Please discard all other forms previously downloaded.
Email submission of all signed forms and documents is preferred but If you choose to submit any materials in hard copy, please submit only single-sided pages.
